Letter of Good Standing Request Form
A Letter of Good Standing is requested by individuals who have been asked to become a Sponsor for a Baptism/Confirmation or to support an application for a Catholic school tuition reduction.
At OLMC it is issued to individuals who:
- have been an officially registered parishioner at OLMC for at least 6 months. This will be followed strictly beginning in January 2027.
- attend Sunday Mass weekly.
- are tithing on a regular basis through Electronic Giving, checks or Tithing Envelopes for cash giving as recorded in our system. (Tithing is a manifestation and act of one’s faith; it is not optional.)
- are active in parish ministry/ministries.
To request a Letter of Good Standing please complete the form here. Turnaround time is 1 week. OLMC Parish Front Office staff will contact you when your letter is ready for pick-up from the parish office at 2121 S Rural Rd, Tempe, AZ 85282 (open on M, W, Th 9:00 A.M. – 5:00 P.M. and Tue 9:00 A.M. – 10:45 A.M. and 12:15 P.M. - 5:00 P.M.)
